That's corny, Rev. changes should be in a legend, or Rev. Block, stating what each Rev. change is added or taken away. Not saying this wrong or right, my best guess on this would be lazy, or uneducated.
I see that Rev change block in the upper right hand corner of most of our prints along with a letter next to where the change was made, but not with a triangle around it.
