We currently fill out a CMM Request Sheet for each part we put on the CMM.

Is it possible to add this information to a Form that comes up at the start of each CMM Program for the details section?

Will then need a Pass / Fail Section at the end of the CMM Program and approval.

I'd like these to appear at the start and end of the CMM Reports

sorry my friend but there are too many possibilities that can be wrong, without more information i can't help you.
is there any code you want to share? from which source are "names" obtained?
filling the two combo boxes is probably the problem?
Again, you'll probably need to use the events to fill the second combo box properly.
Only this time you have to use the event of the first combobox.
